The core difference is that acupuncture is an invasive external treatment method of traditional Chinese medicine, while massage is a non-invasive physical intervention. The depth of the two effects and the range of diseases they can adapt to are quite different.

I have been doing physical therapy for almost ten years. Half of the patients I have dealt with come to massage to relax, and the other half seek acupuncture to treat chronic diseases and relieve pain. A while ago, I met a young man in his 20s who stayed up late and slept with the air conditioner blowing. I first pressed his shoulder and neck for ten minutes, and most of the rock-hard stiffness on his trapezius muscle softened. However, he still felt pain when he tried to turn his head. He said that the pain was so deep that he could not even touch it. Finally, I inserted a needle into the Houxi point on his hand and the Xuanzhong point on his foot. While I was leaving the needle in place, I asked him to slowly move his neck. When the needle was started in ten minutes, he was able to turn the whole angle on the spot and said that the pulling pain was gone.

In fact, this is the difference between the two levels of action. The force of massage mostly acts on the middle and superficial tissues such as skin, fascia, and muscles. Muscle strains, fascial adhesions, and lumps caused by sitting for a long time can be slowly loosened by pushing, kneading, and rolling. After the qi and blood are circulated, it will naturally feel comfortable. However, some problems are stuck in deeper meridians or adhesions in deep muscles. The strength of the hands cannot penetrate, so one can only rely on acupuncture needles to accurately penetrate the corresponding acupuncture points and directly stimulate the target points through the surface tissues. The efficiency of regulating qi and removing blood stasis will be much higher.

There are also different opinions in the industry. Many senior massage therapists believe that as long as the force and angle of the acupuncture points are accurate enough, a qi-refreshing feeling similar to acupuncture can be achieved. I once met an old man who said that when he tapped the Hegu point on my hand, the soreness and swelling went directly to my shoulders, which was almost the same as the feeling of acupuncture. However, this technique requires too much experience of masseurs and is difficult for ordinary consumers to encounter. In addition, for treating chronic diseases related to the viscera, such as long-term spleen and stomach deficiency and allergic rhinitis, acupuncture will still be more targeted in allocating acupoints along the meridians. After all, the stimulation depth of needles is difficult to achieve stably by hand.

Many people will ignore the difference in contraindications between the two. If the skin is damaged or has coagulation disorder, acupuncture is definitely not allowed, but massage can be done in most cases as long as the damaged area is avoided.; On the other hand, if you encounter the edema stage of acute lumbar sprain, heavy pressure will aggravate tissue leakage. At this time, pricking the distal acupuncture points will relieve the pain without touching the injured area at all, but it can help speed up the edema subsidence.

To use an easy-to-understand metaphor, you can compare the meridians to the water supply pipes at home. Massage is equivalent to rubbing from the outside of the water pipes to remove the stains on the outer wall that are lightly clogged. It is suitable for daily relaxation and superficial muscle fatigue.; Acupuncture is equivalent to directly accessing the inside of the water pipe, stirring up the hard lumps stuck in the depths, and adapting to deeper blockages and chronic disease treatment. There is no absolute distinction between the two, but the adaptation situations are different. If your back is sore and your shoulders are tight after sitting for a long time, find a reliable massage parlor and you can relax it for half an hour. ; If it still hurts after being pressed several times, or if the pain is very deep and the specific pain point cannot be touched, then you might as well try acupuncture, which may have different effects.
