Prostatic fluid routine is a method of collecting prostate fluid for laboratory examination through prostate massage, which is mainly used for the diagnosis of prostatitis. It is necessary to abstain from sex for 3-5 days before the examination. During the examination, the secretions of the prostate are obtained by massaging the prostate through digital rectal examination.
The inspection process is divided into a preparation stage and a sampling stage. The preparation phase requires avoiding strenuous exercise or drinking alcohol for 24 hours before the examination, and emptying the bladder and rectum. During the sampling stage, the doctor wears sterile gloves, applies lubricant and performs digital rectal examination. He touches the prostate along the front wall of the rectum with his index finger, and gently presses from both sides toward the central sulcus several times to allow prostatic fluid to overflow from the urethral opening. The specimen is collected with a sterile slide or test tube. If sufficient samples are not obtained during the first massage, the operation can be repeated after an interval of 3 minutes. The obtained samples must be sent for testing within 1 hour to avoid liquid evaporation or component degradation affecting the accuracy of the results.
Routine examination reports of prostate fluid mainly include appearance color, pH, white blood cell count, lecithin bodies and other items. An increase in leukocytes may indicate inflammation, and a decrease in lecithin bodies is common in chronic prostatitis. Some patients may experience temporary perineal swelling and pain or hematospermia after sampling, which usually resolves within 24 hours. Doctors should be informed of recent medication history before sampling, as antibiotic use may interfere with test results. Patients with perianal abscess or severe hemorrhoids should not undergo this examination.
It is recommended to avoid sexual intercourse or riding a bicycle and other behaviors that compress the prostate within 24 hours after the examination. You can take a sitz bath with warm water to relieve discomfort. If the examination shows abnormality, further evaluation should be combined with urine routine, semen analysis or prostate B-ultrasound. After prostatitis is diagnosed, medication should be taken according to the doctor's instructions and reviewed regularly. In daily life, be careful to avoid prostate congestion behaviors such as sitting for long periods of time and holding in urine. Moderate exercise can help improve local blood circulation.
