Is it normal to have a circle of small particles around the glans?

A circle of small particles around the glans may be a normal physiological phenomenon, or it may be a symptom of disease. In most cases, they are pearly penile papules, and a few may be related to diseases such as condyloma acuminata and ectopic sebaceous glands.

Pearly penile papules are a common benign lesion in men. They appear as small white or light red particles arranged neatly on the edge of the glans coronal sulcus, with a diameter of about 1-3 mm, a smooth surface, and no pain or itching. This phenomenon is related to excessive foreskin, poor local hygiene or changes in hormone levels. No special treatment is required, just keep the perineum clean and dry. In some people, the condition may resolve spontaneously as they age.

If the small particles appear cauliflower-shaped, bleed, ulcerate, or grow rapidly, you need to be alert to sexually transmitted diseases such as genital warts. Sebaceous heterotopia is characterized by clusters of light yellow granules, which are mostly related to the strong secretion of androgens after puberty. This type of situation may be accompanied by symptoms such as local itching and abnormal secretions, and a clear diagnosis needs to be made through an acetic acid white test or pathological examination.

It is recommended to avoid scratching or self-medication, wash the foreskin and glans with warm water every day, and avoid using irritating lotions. For patients with excessive foreskin, circumcision may be considered to reduce local irritation. If the particles continue to enlarge, change color, or cause pain, you should promptly go to a dermatology or urology department to rule out pathological factors such as human papillomavirus infection.
